Transcript Request Form When filling out scholarship applications there are two types of transcripts that are requested: 1. Official Transcript: Transcript that is sealed in an official Tidehaven High School envelope. 2. Unofficial Transcript: Non-sealed. Please note: If scholarship application is to be turned into the Counselor's office to be mailed or scholarship sponsor is picking up the application please request an Unofficial Transcript to be enclosed with the application. An Official Transcript is not necessary for this type of application. Click on Transcript for a form. Alumni requesting transcript: Graduates of Tidehaven High School click on "Alumni" for a transcript request. |
