Current or potential vendors of a school district are required to file a
conflict of interest questionnaire with the school's records administrator in accordance
with chapter 176 of the Local Government Code. School districts who maintain a web
site are required to post each vendor questionnaire on the district's Web site.

As of June 29, 2007, the Texas Ethics Commission adopted updated Forms CIS
and CIQ as required by H.B. 1491, 80th Leg., Regular Session, which became
effective May 25, 2007. They can be found at
